Responsibilities:

  • Manage chronic/complex-care members by conducting assessments and developing individualized care plans.
  • Coordinate and monitor care, document progress, and communicate with members/providers, including home or face-to-face visits as needed.
Qualifications:
  • RN license (WA or Compact)
  • 3–5 years Case Management/Insurance CM experience
